Konventionelles Interferon-a und pegyliertes
Interferon-a (kovalent an einen Polyethylenrest gekoppeltes Interferon-a) verbinden hohe Wirksamkeit mit fehlendem leukämogenen und tumorogenen Risiko.

IE Interferon-a subkutan (beziehungsweise 40
µg pegyliertes Interferon-a) pro Woche reduziert es
Erythrozytose, Splenomegalie und Pruritus.

Weder wurde vermehrt Prostata­krebs unter einer Testosteron­substitutions­therapie diagnostiziert, noch besteht bei Männern mit hohen Testosteron­spiegeln eine erhöhte Tumorinzidenz.
Hohes Testosteron wird häufig mit vermehrtem Haarausfall in Verbindung gebracht.
Ein Zusammenhang, der in dieser monokausalen Type nicht besteht.Eine hormonelle Ursache für verstärkten Haarausfall kann allerdings eine Erhöhung des Hormons Dihydrotes­tosteron (DHT) sein.
Neben Monatsspritzen, Tabletten oder Pflastern besteht die Möglichkeit, das natürliche bioidentische Testosteron in Kind einer transdermalen Applikation mittels Gel
täglich auf die Haut aufzubringen. Hierdurch kann mit
einer niedrigst möglichen Dosierung ein sehr konstanter Hormonspiegel aufgebaut werden. Hautpflaster zeigen zwar auch eine gute Wirkung, werden jedoch kaum noch angewendet,

Die Zufuhr über Tabletten ist veraltet, weil
sie keiner natürlichen Substitution, sondern vielmehr einer hoch dosierten Pharmako­therapie entspricht.

Um eine im Vergleich flachere regelmäßiger samenerguss steigert
testosteron - Octavia,-Wirkungskurve zu erreichen, stellten die Baseler Internisten auf das intramuskulär zu applizierende
und länger wirksame Präparat Testosteronundecanoat um.
In Zukunft soll sich der Zeitpunkt der Testosteron-Substitution am klinischen Allgemeinzustand des Patienten sowie am
peripheren Androgenspiegel orientieren. Zu beachten ist außerdem das erhöhte Prostatakarzinom-Risiko unter
der Substitutionstherapie, dem Patient wurde empfohlen, sich regelmäßig klinisch und sonografisch untersuchen lassen. Inzwischen lagen die Ergebnisse der Knochenmarkspunktion vor,
wonach ein normo- bis leicht hyperzelluläres Knochenmark mit leichter Hyperplasie der Erythropoese bestand.

Es fanden sich kein morphologischer Anhalt für eine
myeloproliferative Erkrankung und keine genetischen Aberrationen. "Die preliminary geschilderte abdominale Symptomatik interpretierten wir im Rahmen der Testosteronüberdosierung bei Regredienz im Verlauf als potenzielle Nebenwirkung", so Kofler und Mitarbeiter.


Diese sekundären Polyzythämien sind eine physiologische Antwort auf die entstehende Gewebehypoxie.

Bestimmte Mutationen im Bereich der a- beziehungsweise b-Ketten des Hämoglobins
können zu Hochaffinitäts-Hämoglobinen führen, die Sauerstoff im peripheren Gewebe in verringertem Maße freisetzen. So liegt
bei einem O2-Partialdruck von 20 mm Hg im Kapillarbereich
35 % Oxyhämoglobin vor, wohingegen zum Beispiel beim Hochaffinitäts-Hämoglobin Johnstown noch 60
% oxygeniert sind (Grafik 1). Die dadurch verursachte Verringerung der Sauerstoffabgabe im peripheren Gewebe
führt zu einer kompensatorischen Erhöhung der Hämoglobin-Konzentration. Daneben kann auch
eine Erniedrigung des intraerythrozytären 2,3-Bisphosphoglyceratspiegels durch 2,3-Bisposphoglyceratmutase-Mangel zu einer erhöhten Sauerstoffaffinität des Hämoglobins führen.
Eine seltene, aber bei Männern wichtige Differenzialdiagnose der Polyglobulie ist die testosteronbedingte Polyglobulie.
